Patient Safety Teleconferences
Recognizing the need to offer more learning opportunities to the community, in 2009 the Coalition expanded to host 12 monthly teleconference calls for professionals in the health care community (up from the previous four quarterly calls).
In 2010, we will continue to offer monthly teleconferences covering a wide range of topics and will begin using WebEx technology. In scheduling the presentations and discussions we strive for a balance of topics, speakers, organizations and regions represented.
Effective January 2010, a fee of $40/teleconference line will be charged to non-members participating in the conference calls. There will be no charge to Coalition members.

- August: Sheila Yates, Manager, Patient Safety and Diane Schultz, Medication Safety Manager (Group Health Cooperative, Seattle) spoke about creating a Patient Safety Culture in Ambulatory Care and Why Reporting Counts.
- July: Dr. Hope Wechkin, Medical Director (Evergreen Hospice and Palliative Care) spoke about the safe and appropriate use of hospice and palliative care services.
- June: Rebecca Davis (Harrison Medical Center), Lynnette Truxal, RN, MN (1.1 MB PDF) (Madigan Army Medical Center), and Rosemary Grant (University of Washington) presented an overview of their experiences in improving surgical site infections through use of national and local benchmarking data (i.e., NSQIP and SCOAP).

- April: Jeanette Harris, Infection Preventionist (Multicare Health System), presented on dealing with Multi-Drug Resistant Organisms - Prevention in Seven Steps.
- March: Cindy Sayre, MN, ARNP (University of Washington Medical Center) discussed Fall Prevention -- Challenge and Opportunity.
- January: Norman Gregory (Providence Everett Medical Center) spoke about the Culture of Transparency and Dr. Dan O’Connell shared challenges and strategies for Disclosing Errors and Resolving Adverse Events and Outcomes, including how to support both patients and providers.

- December: Jeanette Harris, MS, MSN, MT(ASCP), CIC (MultiCare Health System, Tacoma) and Betsy Pesek (Overlake Hospital Medical Center, Bellevue) presented two different approaches to preventing catheter-associated urinary tract infections:

- November: Dr. Judith H. Hibbard (Institute for Policy and Research Innovation, University of Oregon) spoke to participants about activating consumers to improve health and reduce costs and Dr. Marc Pierson (St. Joseph Hospital, Bellingham) shared his advice on improving the connections between patients and professionals.

- October: Linda Furkay, PhD, RN, Department of Health Adverse Events Reporting Program Officer, updated us on findings from the Washington State Serious Reportable Events Program (expanded version), including what hospitals are learning from their reports and root cause analyses; and Jan Iverson, RN, MSN, Chief Nursing Officer & Joyce Cardinal, RN, MBA, Director of Quality Management, at Skagit Valley Hospital shared their innovations on preventing pressure ulcers and falls (including pre- and post-fall huddles) and how they use RCAs to learn from near-misses and avoid Never Events.

- August: Cindy Sayre, Director of Professional Practice, Patient and Family Centered Care, University of Washington, School of Medicine, presented on Fall Prevention - Challenge and Opportunity, and provided The Johns Hopkins Hospital Fall Assessment Tool (Modified) for use by participants.
- June: Carol Lyn Vanevenhoven, PharmD, Pharmacy Clinical Operations Assistant Manager/Medication Safety Officer, Yakima Valley Memorial Hospital; Tim Lynch, PharmD, MS, FABC, Pharmaceutical Services Manager, St. Joseph Medical Center; and Jackie Biery, PharmD, Medication Safety Pharmacist, University of Washington Medical Center presented and lead a discussion on the topic of "Managing High-Alert Medications."
Yakima Valley also provided an Insulin Fast Facts handout and UW Medical Center shared a Heparin protocol for use in other organizations. - May: Tracey Jones, RN, Perioperative Educator, Virginia Mason Medical Center; Suzanne Saunders, RN, BSN, CNOR, Staff Development Coordinator, Wenatchee Valley Medical Center; and Linda Goodwin, RNC, Med, Patient Safety and Regulatory Specialist, Evergreen Healthcare presented on the topic of "labeling medications on and off the sterile field."
- April: Trece Gurrad, RN, BSN, Regional Director - Acute Care Services; Jennifer Lefebvre, RN, Clinical Manager Cardiology; and Kandi Davenport, RN, Staff Nurse and Relief Charge Nurse Cardiology, St. John Medical Center, PeaceHealth, Lower Columbia Region, Longview, WA presented on the topic of "Reducing Patient Falls: The St. John Medical Center TCAB Experience."
- March: Island Hospital discussed their Progress Report one year after implementing Bedside Bar-Coding, including implementation, tracking outcomes, use of work-arounds, and positive and negative aspects of the application.
- February: Kathy Mertens, RN, MN, MPH, Director of Clinical Care Systems, and Paula Minton-Foltz, MSN, RN, Assistant Adm. Education, Quality & Patient Safety at Harborview Medical Center shared processes, challenges, what has worked or not, advice, lessons learned, and tools regarding the use of patient identifiers in the inpatient and outpatient settings.
- January: Connie Kirkpatrick at Good Samaritan Hospital, Puyallup, has provided Notes on Care of the Oversize Patient, and Airlift Northwest has provided a copy of its Large Patient Air Transport form. North Valley Hospital District has provided a copy of its Bariatric Admission Guidelines for Special Needs Patients; for more information, contact Kathy Swedberg, RN, their Quality Risk Manager.
