Imagine being 70 years old, living with 5 chronic conditions, having 37 healthcare-related visits annually with 14 different providers, scattered over different locations. Navigating a complex healthcare system is reality. Research shows that moments of transitioning care from one facility, unit or provider to another are some of the most vulnerable times for our patients, filled with opportunities and omission. This one-day course features Dr. Eric Coleman, a Geriatrician and national expert on Care Transitions from the University of Colorado, Denver. Dr. Coleman functions in an advisory role to the IOM, CMC/QIOs, MedPAC, NQF, IHI, and the Joint Commission. He also coauthored section 3026 of the recent health reform law.
This course is broad in its timely scope, essential for RNs, MD, ARNPs, MSWs, CHPQs, as well as patients and their family caregivers.
